If you visit the companion website for Production Management for Television written by Leslie Mitchell, you will find a wealth of useful forms to be used during the production process for a TV programme. Many of these may be similar to those used for film as well. With one exception every form on this site is illustrated and explained in the book Production Management for Television. Some of the forms are so simple they need virtually no explanation. Others, however, will be unintelligible without reading the text and I can only suggest that if you do not already have it and need help, you obtain a copy of the book and look up the relevant chapter.
